Authorities identify second person killed in industrial accident

Authorities have identified the second person killed in a fire and building collapse at International Nutrition.

The body of 47-year-old David Ball was recovered Tuesday afternoon from the second floor of the industrial plant. 53-year-old Keith Everett was also killed. Ten people were injured. The fire and building collapse happened Monday morning at the south Omaha manufacturer of food and vitamin supplements for animals.

Meanwhile, Occupational Safety and Health Administration officials have arrived at the site and will take over the accident investigation. The city of Omaha Planning Department declared the building unsafe and dangerous. The second and third floors of the International Nutrition building collapsed on to the first floor.

In a statement, International Nutrition president Steven Silver says “In our more than 35 years of doing business, this is the most serious incident to ever happen at our operation, and we are fully cooperating with all government investigators to find the cause.”
